Sarasota SO Authorization for Release of Protected Health Information
The Sarasota, FL Sheriff’s Office has asked doctors who prescribe pain medications to have their patients sign an authorization form giving the doctor the authority to break doctor/patient confidentiality and talk to law enforcement and give them records concerning the patient’s pain medication usage.
